The first question to ask yourself: what is my situation?

The breast pump aisle is overwhelming. Dozens of models, prices ranging from low to high, and marketing claims that all sound the same. This guide is based on what Lauriane, an IBCLC-certified lactation consultant, and Marion, a product manager at Fizimed and PhD in neuroscience (with 4 years of cumulative breastfeeding experience and dozens of models tested), explain to the mothers they support. Practical criteria to choose what will truly suit your situation, and the right techniques to use it effectively.

There is no such thing as a universal "best" breast pump. There is only the best breast pump for your situation. And that situation will evolve over the weeks and months. Lauriane emphasizes this point: choosing a breast pump must first answer a simple question: "Why do I need it, and at what stage of my breastfeeding journey am I?"

The 4 types of breast pumps and their use cases

The manual breast pump

Simple, no motor, no battery. You create the suction pressure yourself with a manual pump. It slips into a bag, requires no electricity, and is very easy to clean.

It is a good choice for very occasional use: to relieve occasional engorgement, to pump a little milk for a bottle, or as a supplement to an electric pump while traveling. However, it is not suitable for intensive or regular use: the physical fatigue sets in quickly and the power remains limited.

The hospital-grade electric breast pump

This is the gold standard whenever you need to pump regularly, intensively, or in delicate situations. The suction power (300 mmHg and above) and cycles (between 60 and 120 per minute) allow for drainage comparable to the suction of a baby well-positioned at the breast.

Lauriane is very clear: for starting breastfeeding with a pump, separation from a newborn (prematurity, hospitalization), relactation, or boosting a low milk supply, it has to be a hospital-grade pump—and only that. In France, these are obtained via prescription at a pharmacy as a rental. It is a regulated medical device, which guarantees compliance with strict power and safety standards.

The wearable (portable) breast pump

This is the breast pump that slips directly into your nursing bra, with no wires and no external motor. It allows you to pump while standing, in a meeting, on the train, or at the office, without having to undress or isolate yourself.

Marion explains it with a frankness worth noting: the first generations of wearable breast pumps were not very effective. They didn't maintain their power over time, reservoirs would overflow, and Bluetooth connections caused issues. Today, some models have improved significantly. Between the rented hospital-grade pump and the portable wearable, there are personal electric corded breast pumps, which are often available for purchase. They are well-suited for regular home use by a mother whose breastfeeding journey is well-established.

The flange: the make-or-break factor

If you only remember one thing from this guide, let it be this: flange size is the most critical factor for the effectiveness of your pumping session. Much more than the brand of the pump, and much more than the price.

Why the standard size often doesn't work

In France, the standard flange size provided in most maternity kits is 24 mm. However, the majority of women have nipples between 13 and 15 mm. The gap is significant.

A flange that is too large pulls part of the areola into the tunnel. This suction of areolar tissue reduces drainage efficiency, causes pain, and can eventually lead to edema. Conversely, a flange that is too small causes the nipple to rub against the walls, which leads to cracks and blocks drainage.

The result in both cases: less milk pumped, more pain, and often a mother who thinks she isn't producing enough when the real problem is an ill-fitting flange.

How to measure and choose your size

The method recommended by Lauriane: measure the base of the nipple and the tip of the nipple. The difference is generally 2 mm. Use the middle millimeter as a starting point for choosing your size.

But measurement alone isn't enough. It is the dynamic measurement—actually testing the flange on the breast—that provides the real answer. The criteria for success are twofold: the comfort felt by the mother (no pain or rubbing sensation), and the appearance of milk drops within the first few minutes.

This measurement can be done antenatally, even before giving birth. Lauriane highly recommends it: arriving at the maternity ward with the right flange sizes prevents you from spending the first few days pumping with unsuitable equipment.

Flange shape and material

Flanges come in different shapes (flatter, pointier, rounder) and materials (rigid plastic or soft silicone). Soft silicone generally offers more comfort and better adaptation to your anatomy. It also allows for a slight margin of tolerance (about 1 mm) that does not exist with rigid plastic.

Marion makes an important point: when you add an insert from another brand to a breast pump, the manufacturer can no longer guarantee the device's effectiveness or that there won't be a loss of suction. That is why having a range of sizes within the same brand, tested and validated together, is preferable. Fizimed offers six flange sizes precisely to avoid this issue.

Suction modes: mimicking a baby at the breast

A good breast pump replicates two distinct phases:

The stimulation phase : rapid, light suction that mimics a baby's initial suckling when they are "calling" for milk. This is what triggers the let-down reflex.

The expression phase : slower, deeper suction that matches the baby's rhythm once the milk is flowing. This effectively drains the available milk.

This alternation is fundamental. A breast pump that only offers one mode or one intensity does not accurately replicate the natural mechanism. Lauriane recommends being able to adapt the modes to your own body and let-down reflexes, rather than being stuck with a single automatic mode.

Marion adds a practical point based on feedback from Fizimed moms: the pause function is highly valued. When using a single breast pump to pump both breasts alternately, being able to pause without losing your settings is a real time-saver.

Daily use: the most common mistakes

Increasing intensity to get more milk

This is the most common mistake. Moms think that pumping harder will produce more milk. That is false. Intensity does not increase production; it can actually cause pain and trigger a negative association with the breast pump, making sessions less and less effective. What increases production is the frequency of stimulation.

Pumping for too long or not long enough

An effective session lasts between 15 and 30 minutes, depending on the mother's let-down reflexes. Some moms have one let-down reflex per session, while others have two or three. The duration should be based on your body, not a fixed timer. If nothing happens after 5 minutes, gently massage your breasts and wait for the let-down reflex before stopping.

Ignoring signs of pain

If pumping is painful, there is a problem. It is never normal and should never be "accepted." The most common causes: a flange that is the wrong size or shape, intensity that is too high, or a breast pump that isn't powerful enough, requiring you to compensate with excessive intensity. In any case, the solution is to identify the cause, not to endure it.

Neglecting cleaning

Cleaning after every session is non-negotiable, especially for parts that come into contact with milk. A breast pump with parts that are difficult to disassemble or clean will gradually be used less, or used in a less hygienic way.
